School support request planner

1. Contact the school and education directorate for the current form and procedure.
2. Ask the school which assessment and documents apply to this support type. Procedures differ.
3. Confirm with the school whether this is a new request or renewal.
4. Circulars and forms change each school year. Ask for the current circular; do not reuse another year's dates.
5. This does not decide entitlement, diagnosis or placement. Do not enter a child's name or health information.

Confirm the required supporting document with the school before applying.
